To combat its growing diabetes problem, the State of Colorado Department of Health Care Policy and Financing worked with McKesson Health Solutions to operate a Diabetes Disease Management Program funded by Lilly.
The program involved telephone- or community-based registered nurses who contacted Colorado Medicaid beneficiaries to provide disease education and support to the program's participants.
A 12-month analysis of the program showed:¹
- A 69-percent increase in the number of patients who have an action plan for dealing with their disease
- A 43-percent increase in the number of patients taking aspirin or a certain drug to help prevent heart disease (diabetes is a major risk factor for heart disease)
- A 33-percent increase in the number of patients who know their cholesterol level
- 95 percent of responding patients reported satisfaction with the program
- 94 percent would recommend the program to a friend or family member with diabetes
